A personal data questionnaire – is to be submitted with the following attachments:

  1. Curriculum Vitae;
  2. certified copy of the diploma of completion of a second-cycle degree programme or a uniform master's degree programme; this year’s graduates who do not yet have a certified copy of their diploma submit a certificate that they have successfully passed their diploma examination (valid for 3 months);
  3. index book of a completed second-cycle degree programme or a uniform master's degree programme, or a supplement;
  4. English language certificate (at least B2 level) or other equivalent document unless the documents stipulated in Art. 1 point 6 item 4 of The Terms of Admission to the CUT Doctoral School contain information on the level of linguistic competence in English;
  5. preliminary Individual Research Plan approved by a prospective promoter (docx, pdf);
  6. recommendation written by a prospective promoter, who is a senior academic staff member, approved by the Dean or the Head of the Scientific Council of the Discipline in question (docx, pdf);
  7. list of documented scientific accomplishments (publications, papers delivered at national and international conferences, other academic papers);

  9. certificate of completion of a professional internship (applies to candidates in the discipline of architecture and urban sciences - pdf);
  10. declaration on the doctoral scholarhip (pdf);
  11. a colour photograph – in the electronic version – is to be attached when making the electronic registration; candidates are required to attach a file containing the electronic version of a photo (for a doctoral candidate ID): in JPG format; dimensions: 236x295 pixels ± 3 pixels; the photograph should comply with the requirements applicable to photographs used in personal identification documents).
